- UPLOADED DOCUMENTS SUCH AS A HIGH SCHOOL DIPLOMA/GED OR A VALID LOUISIANA TEACHING CERTIFICATE
- THREE (3) COMPLETED REFERENCE SURVEYS FROM NON-FAMILY MEMBERS
